Most ortho coverage has a lifetime (as opposed to a yearly) maximum - this varies from $750 to $2000. This benefit is separate from the restorative and preventative maximums which usually renew each year.

Finding individual ortho plans is difficult, however most orthodontists take payment plans. They can afford to take payments because your care is not finished in one visit and they know you will continue to come in regularly.
