Answers:
around my area- dentist do not use the strongest cement available to seat crowns on the implant abutments just in case something ever happens to the implant.

Not common, but does happen. I know that it does not feel good.
If the work is not done properly, yes
Are teeth bones? if not what are they.?
Well, the crown is either put on an abutment that's screwed into the implant or the crown is attached by a screw.Either ways there's a screw holding the crown, and screws do get loos sometimes (6 - 12 % of single implants experience that problem according to research).
Also rarely the crown only come out from the abutment if it is cemented on it which also happen.
It's a simple problem .. don't worry.. ok!!
