Where can I get dry ice?

Answers:
Try calling the dental office and resheduling appointment for sooner because you're in pain. It can get much worse, and it can become life-threatening. You have an infection going on 4 inches away from your brain. That's not a joke.

Dentist can remove any tooth in any condition. Cost may vary depending on the difficulty of the procedure.

If it is your permanent tooth (2nd set) you might want to ask your dentists about possibilities of repairing it. Once it's out, it's forever.

does anybody know any good teeth whitening products that work?

Firstly, do you not see a dentist 2xyear, and why not?
Secondly, he/she may not be able to save it
3rdly, if you do not have a bridge made where that tooth was (because by the sound of it, it looks like it is a gonner, and will have to be pulled) the opposing tooth will be lost in the years to come, as will the two beside it. Teeth are dynamic -- that means that they move throughout your entire life. If one wears braces, for example, and does not continue to wear a retainer, in 10 or 20 or more years, they will go back to where they were. Sorry for you. You only get one set of secondary teeth.... you should change your thinking and take care of them....

Does anyone know of a good orthodontist in the NYC area that is reasonably priced?

It may not be the easiest tooth to take out, but it can be done.

if you have braces and drink/eat lots of caffine will the braces leave holes?

yes, it can be pulled. It may have to be done by an oral surgeon which may cost a little more. I had a couple in the back pulled several years ago and the surrounding teeth are fine. In fact I go on the 22nd to get another pulled on the other side. I've heard people say that if you put and asprin on the tooth and let it disolve it will ease the pain. That sounds like torture to me, so I've never tried it. Don't worry about those who say you haven't taken proper care of your teeth. They just don't understand poor genetics and circumstance. Good luck and take care.
