As per a Law regarding Health Care, HIPAA (Health Insurance Portablity and Acountablity Act) was created to give patient rights as to where and how the patient's medical records were handled. I.E. Only the referring Dr., referred Dr., 3rd party lab (which must be even more strict), your insurance company(s), and any other person designated by the patient (must have a waiver form filled out by that patient if it is not a legal guardian of a patient under 18) can legally see your records. By not signing the reciept, they may or may not treat you as they could not send the patient records to a place that could really help. Or, it could end up costing you more $$ as your insurance company may deny the claim based on lack of info. that could not be sent. Working in a Dental office for 2 years, I know that it is safe to sign the form and you are not signing your life away.

You are signing the policy in place in that office has regarding the privacy laws now in place. What this means is that that cannot give out any medical information to any person unless you have authorized it. In other words they can't tell anyone about your medical, in this case dental, history, current treatments or status. No big deal,for your protection only.
That is the Hippa Policy.....mandated since last year. It just says that you have allowed them to "invade" your dental pricacy....NO BIG DEAL....It doesn't go anywhere, do anything, just policy "red tape"..

Actually, this is a staement about your privacy rights. It is to let you know, that we at the dental office, cannot release any of your medical or dental, or personal information, to anyone accept to persons aiding in your treatment. Such persons would be your insurance company, a referring doctor, or a lab that would help make any of your work. This has always been how things have worked, just since 2003, they have made us at the dental office, have you the patient, sign a form. We must keep this in your chart. You do have the right to revoke letting them give any information. However, be aware that if you do not agree to the form, any office can refuse to see you. Good luck. Its really no big deal.
