Do the enamel of your teeth grow back once it is worn out, as in from sugar and plaque?

enamel doesn't really come back no, but there are tooth pastes that help the enamel..and harden your teeth..I use Luminous by Colgate, it is a enamel strengthener. Also you can go to the dentist, and get your teeth bonded, or a preocess similar..expecialy if your enamel is gone it will help keep your teeth much longer.

I use to smoke for many years, and my teeth remained white. It was whiter than most people. I took pride in keeping it white..

demineralised enamel can get remineralised with the help of fluoride. but once the enamel breaks off, it can never grow back. so if a cavity is already present, its best to get ur tooth filled rather then to wait and let the cavity get bigger n more damage is done.
