It depends on why you don't lose them. Some people are born and are naturally missing a few permanent (adult) teeth. If they don't form, the deciduous (baby) teeth that they should replace will often remain much longer than usual. I know of several people in their 50's with a baby tooth or two still present. They tend to wear down and become brittle and often end up having to be taken out at some point. If the baby teeth AND the adult teeth both come in, usually there is too much crowding and the baby will teeth will have to be removed.

If this happened, you would have very small teeth and probably be in a lot of pain from the impaction of your adult teeth. You'd have to see a dentist and get the baby teeth pulled, I'd assume.
