The build-up comes from plaque, but its actually called tartar. The actual damage to the enamel (surface of the tooth) comes from the plaque, it produces acid that can cause decay. Theres not much that can be done at this point, since it has built up over time, however, at this cleaning, they can (will) remove the tartar from the enamel. The build-up is called tartar or calculus. It's what happens when plaque gets left on your teeth and it hardens. The only way to remove it is by a dentist or hygienist. If it is not removed, it could cause decay in his enamel. However, with regular cleanings, he shouldn't have anything to worry about.
