Does anyone know where their local municipality gets the fluoride from to put in the water?

In Massachusetts, the Department of Environmental Quality Engineering (DEQE) is the agency responsible for the public supply water works. You could contact the Department of Public Health or your Local Water Department Superintendent.
Fluorides are added to the water as liquids or solids via solution feeders or dry feeders. The compounds most commonly used are Sodium Fluoride (most common and is available as minute crystals) Hydrofluosilicic acid (a liquid) or Sodium silicofluoride (a crystalline powder).
I believe that local water works are required to keep daily fluoride records.

They buy it is liquid form from a local distributor. Each does their research and buys it from a local supplier, usually thru a bid process. Your local Government people can get you the info.
there are 2 sources i know of... 1) from natural fluoro-oxide-somthing deposits / by-products of something i cannot remember.. or 2) powder or liquid forms manufactured by factories for other uses such as toothpaste and washing detergents.
