I usually recommend every 2-3 months. If you have the power type brushes you might be able to go 4 months before changing the heads. Some brushes have an indicator on them that fades when the brush is getting too worn. Soft brushes require longer brush time (2-3 minutes), and if you try to keep your soft brush too long the brush time would then need to be longer. I recommend to change the brush if you've been sick, or at least soak it over night in Listerine.

You should always use a soft bristled toothbrush. These will last for only 3-6 months. The toothpaste you use on it should stop most microorganisms from proliferating on it, as long as the dentifrice contains sodium laurel sulfate. Using a toothpaste that contains triclosan (Colgate Total) can aid in killing bacteria in your mouth and on the brush. Running the brush through a dishwasher can help maintain a bacteria-free brush for quite some time, but still can't make it last for more than 6 months.
I have heard that when the bristles start to get worn out you should replace them. I'm sorry I don't know what time period but I know that when the bristles start to fray out to the side then it is time to get a new toothbrush. Hope I helped!
