possibly because you're underage, you shouldn't smoke.

Doctors and dentists are required by Law to keep your information private even if you are a minor unless they feel you are in imminent danger from yourself. You can read about your rights if you do a search for HIPPA laws in your state.
You don't have to make up a story. Just be honest. Tell them you smoke, but you do not want your parents to know. They will keep it confidential. If you are still concerned, call and ask. They will tell you the truth. They can't (and have no reason to) lie to you about it. It is important for them to know if you need any type of procedure done so they can give you proper instuctions.
