Aside from obviously brushing your teeth, flossing and rinsing, try brushing your tongue. That is the area that holds the most odor-causing bacteria, so it makes sense to scrub it good everyday. There are special toothbrushes with the cleaner on the back of the head, but I find that using the bristles is far more effective. It may take a while to get over your gag-reflex, but keep at it. In no time, your wife will be kissing all over your sweet-smelling mouth!
You might have to go to the doctor/dentist and see what they can prescribe to you.

Listerine, good flossing and brushing 2-3 times a day may help to improve your breath. You should talk to a dentist, because you may have a gum disease from lack of proper care. Other then that I would carry breath mints with you, to help freshen your breath.
