Not completely sure what you're looking for; however, having worked in the health system for years - there is a national database of health information - each time your information is provided, which is becoming more commonly linked by computers to be utilized from your local doc to your local hospital and to tranfer your "electronic file" to a "specialist", it allows the CDC to monitor information that might help pinpoint potential risks or outbreaks in areas of various diseases or strains of things that might be new to the USA. Unfortunately it does take an attorney and court order to get the information in your "national" medical file, if one is established on you = not everyone has one, contrary to popular rumor.
