what medicine to take?

Question: I have lack of thought. an impediment to think. Also I cannot in my mind put my self seeing everything that is exterior to me as so and so think about it .I am seeing a psyquiatrist for two years but it has not help me.I do not know what to do. What illness do I have and what should I take?
