Why does it hurt so much to get hit in the testicles?

My guess would be a larger than usual amount of nerve endings in the area.similar to your nose.

Your nerve endings are more sensitive there than anywhere else in your body. It'd be like getting hit on the bottom of your foot or under your arm pit, your neck, or your face -- the nerves there are more sensitive and therefore more susceptible to pain. (It's also the same reason why you're more ticklish there than in other parts of your body.)

Because there's nothing protecting your most sensitive area.

If you wear a cup and get hit it's not a big deal because you're protected.
