try going to a different doctor. the one you have is obviously a moron. my friend had a problem like that and it got so bad that she started taking birth control pills. I think that it worked.

Go to the ER and tell them you are in serious pain they may give you something for it. I have bad cramps too and sometimes I have had to go to the ER before cause they are so bad.
Try Kava Kave or Valerium to calm you. It may ease the pain. Then take a advil, because that is an anti inflamitory.

If you have cramps down there, then your muscles are obviously tightening and causing pain and pulling on your low back causing back pain as well. You can pop an advil or any other meds, or you can do what I do...apply heat. If you don't have a heating pad, get an empty glass bottle and fill it with scalding hot water. rap a towel around it (not too thick or you won't feel the heat), and place it on your lower belly. make sure it's hot enough. If it just feels warm, peel a piece of the towel off.
Also, the food you eat may be causing your cramps. Avoid salty things, or creamy things like ice cream before your period. Those foods just make your cramps worse.
