Although I have heard that people who do it for the first time can have a negative reaction to it including vomiting, so it is possible. The other possibility is that it may have been cut with something else, and that is what is causing the fever. Is this a fever as in the person's temp. has been taken and they are above normal, or are they feeling hot and flushed and just feel hot? No Heroin by itself should not cause a fever.What will cause a fever is the practice of filtering the heroin through cotton balls or cigarette filters. Tiny fibres from the these "filters" are accidentally drawn up into the syringe and injected into a vein. The body will attack these foreign substances as if they are bacteria. This is what causes fevers and chills. It is known as "cotton fever" in the Injection Drug Use community.

Also people crap a lot when withdrawling because when your on heroin I dont know specifically (scientifically) it backs up your intestines. But pure heroin itself doesn't have as bad of a withdrawl as street heroin because once its on the street it gets cut with many other things such as: Guinine, fentanyl (which is what is killing so many people not the heroin), milk powder, and others. Fentanyl is 80 times more powerful than heroin, and is the reason people are dying because it is so easy to overdose not knowing it is in the dose.
