I've used 100 proof vodka, Everclear, a grain alcohol which is is about 190 proof, and vinegar. It depends on whether or not I'm mixing the end result into something else, taking it straight, or using it externally. For example, I use Everclear to make a hoarhound tincture, I then mix a few drops into a tablespoon of honey for soothing a sore throat. Everclear is also what I use if I want to store the tincture long term. Vinegar is good for external applications, especially if it is something that shouldn't be ingested, because there's no mistaking the smell, it too will store long term. It also has beneficial effects itself, I make sage cider vinegar for rinsing my (dark brown) hair. Vinegar strips away shampoo buildup, keeps the scalp in great shape, and is a very effective conditioner even without added herbals.
I use fresh herbs whenever I can get them, you may lose some of the more volatile compounds in the drying process. I use clamp top glass jars, pack the jar full, and then fill with the appropriate extractant. I check and top the liquid off the following day. Leafy stuff gets a week, woody stuff, at least 2 weeks, somewhere out of the light.

Fresh is best, but dried will work. The big difference is going to be the % of alcohol that you use. If you make tinctures with fresh herbs you almost have to use grain alcohol(190 proof). If they are dried you can use vodka (90 proof).
Also, because the dried herbs are more compact, the tincture may be "stronger". Fresh herb is bulkier and you just can't fit as much in a jar.
